How to build a good 360 questionnaire

  1. Keep it short: 15 to 25 rating questions take reviewers about 10 minutes, and short surveys get more honest answers.
  2. Ask about behaviours people can see (“shares information without being asked”), not personality traits (“is a team player”).
  3. Use one rating scale throughout, and give reviewers a “haven’t seen this” option so they don’t guess.
  4. Add two or three open-ended questions: the comments are often the most useful part of the report.
  5. Ask 5 to 10 people from different sides (manager, peers, direct reports, clients) and keep answers anonymous.

Frequently asked questions

Is the questionnaire builder free?

Yes. No sign-up, no limits. You can copy, download and print as many questionnaires as you like.

How many questions should a 360 feedback questionnaire have?

Usually 15 to 25 rating questions plus 2 or 3 open-ended ones. More than 40 questions tires reviewers and lowers the quality of the answers.

Can I edit the questions?

Yes. Download the questionnaire for Word and edit it freely, or use it in PRO, where you can change, add or remove statements before sending it.

Where do the questions come from?

From our free guides of 360 questions by role, written around behaviours that colleagues can observe. Each role page explains the questions and gives example comments.

How do I send the questionnaire and collect answers?

You can paste it into a survey tool, or use our free 360: reviewers answer anonymously by link, without an account, and you get an AI report with strengths, blind spots and suggestions.
