360 Feedback Questions for Nurses
17 ready-to-use questions, example comments and tips — plus a free tool that runs the survey and writes the AI report for you.
Nursing is team work under pressure, and the people who see a nurse's practice best are the colleagues on the same shift. A 360 brings together views from fellow nurses, physicians, allied health staff and the charge nurse, so feedback is fair and specific. These questions focus on behaviours colleagues can observe, not on clinical audits.
Patient care
- How well does this nurse treat patients and families with respect and empathy?
- How clearly do they explain care and procedures to patients?
- How consistently do they advocate for patients' needs and comfort?
- How well do they stay calm and focused in stressful situations?
Teamwork
- How willingly does this nurse help colleagues when the unit is busy?
- How well do they work with physicians and allied health professionals?
- How respectful are they toward all members of the team?
- How well do they support new or less experienced staff?
Communication and handover
- How complete and clear are this nurse's handovers?
- How effectively do they speak up when they have a concern?
- How well do they keep colleagues informed about changes in a patient's condition?
Safety and professionalism
- How consistently do they follow safety protocols, even under time pressure?
- How well do they organise and prioritise their workload during a shift?
- How open are they to feedback and learning from mistakes?
Open-ended questions
- What does this nurse do that makes the team stronger?
- What is one thing they could do to improve patient care or teamwork?
- Describe a situation where this nurse made a difference for a patient or colleague.
Example feedback comments
Good comments are specific, describe behaviour rather than personality, and explain the impact. A few examples:
Strength
“During a very busy night shift she noticed a newer colleague falling behind and quietly took two of his patients' medication rounds. Nobody had to ask.”
Growth area
“Handovers are sometimes rushed at the end of a long shift. Using the same short structure every time would make sure nothing is missed.”

Tips for a useful 360
- Keep the survey short (10–15 questions): reviewers answer between shifts.
- Include colleagues from different shifts and professions, not only fellow nurses.
- Make clear the 360 is for development, separate from clinical incident reviews.
Frequently asked questions
Who should give 360 feedback to a nurse?
Fellow nurses on the same unit, the charge nurse or nurse manager, and two or three physicians or allied health professionals who work with them regularly.
Is a 360 review the same as a clinical competency assessment?
No. A competency assessment checks clinical skills against a standard. A 360 collects colleagues' views on behaviours such as teamwork, communication and professionalism, and works best alongside it.
